  •  If you are 55 years of age or older
  •  Have no prescription drug coverage
  •  Have a chronic medical condition
  •  If you are a resident of Alabama
  •  If your annual household income is less than:

              * $20,420/ 1 person     * $27,380/ 2 people

  •  You are in the Medicare Part D Coverage Gap
  •  Disabled individuals of any age in the 24 month Medicare waiting period
